Employee Dies Falling Through Duct Work

On March 6, 2012, Employee #1 fell down a vertical portion of a 10 ft diameter metal exhaust duct system. Employee #1 was working with a coworker to fit together sections of the exhaust duct system approximately 132 ft away from the unprotected edge of the vertical portion. It was Employee #1's first day working in the exhaust duct system. Employee #1 and a coworker accessed from the exhaust duct system using a hatch midway between their working location, and the unprotected edge of the hatch. There was no artificial lighting provided leading up to the unprotected edge. There was no fall protection method provided at the unprotected edge. At this point, the coworker left Employee #1 at their assigned work location, to adjust his welding machine which was located on the ground level. A little while later, Employee #1's body was found at the bottom of the vertical portion of the exhaust duct system. There were no witnesses to the accident. Employee #1's right arm was nearly severed at the elbow and he suffered massive head trauma during the fall which resulted in his death on site. Employee #1's body was taken by ambulance to a local hospital were he was pronounced dead.
