Investigation Summary

On December 30, 2013, Employee #1 was working as a truck driver. He was employed by a trucking company that moved freight for customers. The incident occurred at a job site in Santa Fe Springs, California. On the day of the incident, Employee #1 reported to work around 7:40 a.m. He inspected his truck and noticed he had a flat tire. After having the truck repaired, Employee #1 received an assignment to deliver a container load to an outfit in Santa Fe Springs, California. Employee #1 left the location at approximately 10:00 a.m. and arrived at the destination around 10:30 a.m. After the truck was unloaded, Employee #1 called his dispatcher to find out where to deliver the empty container. The dispatcher informed Employee #1 to take the container to Pier 400 in Long Beach, California and leave it there. Around 12:40 p.m., Employee #1 called his dispatcher for another assignment. The dispatcher informed Employee #1 to come back to the yard and pick up another container for delivery to the same outfit in Santa Fe Springs to which he had made his earlier delivery. Around the same time, a coworker was directed to deliver a container to the same outfit as well. Employee #1 and his coworker arrived at the destination around the same time. From the street, the coworker motioned to Employee #1 to go ahead. While the coworker waited on the street to complete paper work, Employee #1 docked at Door Number 14 and had warehouse workers start unloading his container. After five to ten minutes, the coworker docked at Door Number 13. As he was backing up, he stopped his trailer approximately 15.2 meters (50 feet) from the dock. The coworker got out of his truck and moved the container on his trailer to the back end of the trailer. This act allowed the dock workers to unload the container from the dock. The container was 6.1 meters (20 feet) long and did not extend the entire length of the trailer. As the coworker was getting out of his truck, he was able to see that Employee #1 was still in his truck as Employee #1's trailer was being unloaded. Further, while moving the container to the back of his trailer, the coworker did not observe any other workers in the area. The coworker got back into his truck and backed the trailer into Dock Number 13. Employee #1, though, was standing by the loading dock. The coworker backed into Employee #1 and crushed him between the loading dock and the truck. When the coworker got out of his truck to see how far away he was from the dock, he observed Employee #1 crushed between his trailer and the dock. Paramedics were called. Employee #1 was pronounced dead, and the Los Angeles Coroner's office took the body. On December 30, 2013, the Cal/OSHA West Covina District office received the report of the fatality from the Santa Fe Springs Fire Department.
