Investigation Summary

At approximately 4:30 p.m. on December 13, 2013, Employee #1, a 56-year-old male, independent contractor (truck-driver) working for South Green Transportation Inc., was operating a 2008, semi-truck manufactured by Freightliner with California license plate number: 9B13717. Employee #1 was pulling an empty 20-ft shipping container out of an in-ground dock. Employee #1 coupled the empty container to the semi-truck but left the landing gear in the down position. As Employee #1 began to pull the shipping container out of the dock the landing gear made contact with the ground preventing the vehicle from moving forward. Employee #1 exited the cab of the truck to manually raise the landing gear while the vehicle was on the incline Employee #1 did not set the air brakes. As a result, the semi-truck trailer began to roll backwards once the landing gear was no longer in contact with the ground. Employee #1 attempted to enter the cab of the vehicle while in motion. The semi-trailer struck the rear of the loading dock throwing the driver to the ground in front of the rear axle. The semi-truck trailer bounced back rolling over and coming to rest on top of Employee #1. Employee #1 was killed in the rollover event. The case was closed as "no jurisdiction."
