Investigation Summary
On July 11, 2013, Employee #1 was cleaning a positive flow pump in Deck A of a production area. Employee #1 used his right index finger to check for left over residue inside the wall of the pump's pipe when it contacted the rotating blade propeller inside the pump. The pump was on and had not been deenergized. Employee #1 suffered an amputation to his right index finger. He was admitted to a local hospital for surgery due to the incident. After the surgery, Employee #1 was released from the hospital.
