Employee's Hand Is Pulled Into Machine, Receives Amputation

At approximately 2:15 p.m. on July 3, 2012, Employee #1 was operating a rolling machine in the coupler department of a facility that manufactures custom extrusion for the housing and construction industry . He was extending a 6 in. round by 8 in. in length piece of metal pipe with the rolling machine. The rolling machine was used to place a crease on the middle section of the pipe. The operation began with the insertion of the piping into the machine through an opening and the movement of a handle to the left to close the opening. After completion of the operation, the handle was moved to the right and another piece of pipe was inserted. The drive roller was spinning while this activity was taking place. As Employee #1 was about to release the cycled part, a burr caught his glove. The burr went through his glove into his right hand. The die automatically dropped. E1 was unable to stop the machine because the controls were on the right side and he could not reach that far. He was wearing Kevlar gloves. As soon as the machine stopped and released, Employee #1 pulled his right hand out. He sustained an unspecified amputation. A coworker gave him some rags to stop the bleeding, and Employee #1 ran to the office to report the incident. He was transported to a medical center, where he received treatment and was then hospitalized.
