Investigation Summary
At approximately 5:10 p.m. on June 9, 2012, Nonemployee was removing dead palm fronds from a palm tree at a residence location. He was at approximate height of 35 ft on the palm tree. As he worked, overhead palm fronds collapsed and enveloped him. Emergency medical personnel were summoned. They responded and reached the scene within 10 minutes. Upon arrival, he was still alive. At approximately 5:50 p.m., Employee #1 was retrieved from the palm tree and pronounced dead.
