Investigation Summary
At approximately 11:45 p.m. on January 10, 2012, Employee #1, was walking across the street and after retrieving a piece of equipment (a disconnect) he was hit and killed by a vehicle. He was a traffic signal installation worker for Southwest Signal, Inc. in Tampa, FL. He was one of a two man crew installing traffic signal lights. While the worker was observed wearing a high visibility vest earlier in the work shift, he was not wearing the vest at the time of the accident.
