Employee Amputates Finger in Machine

On August 5, 2011, Employee #1 was replacing the universal joint in a machine on the Capper Line Number 15. He did not lockout the machine while working on it. Employee #1 placed his right index finger into a small hole on the machine, where the vane inside the rotary was still spinning. The rotating part amputated his index finger at first digit. Employee #1 was transported to a medical center, where he was treated and released.
