Investigation Summary
On November 21, 2011, Employee #1 was cleaning a Nissui S cutter and grinder. With the machine energized, he opened it up to reveal the blades. When Employee #1's left hand hit the safety switch, the grinder amputated his little finger and a portion of his right hand.
