Employee Dies from Fall from Extension Ladder

At approximately 9:30 a.m. on January 26, 2012, Employee # 1, the Crew Foreman, and a second iron worker were installing and bolting steel girt approximately 21 feet high on the front side of a prefabricated steel building. Employee #1 was working from a 24 foot extension ladder that was set at 20 feet high. Employee # 1 fell off the extension ladder to the ground. Employee # 1 was transported to the hospital with severe injuries and later died.
