Investigation Summary
At approximately 5:26 a.m. on January 21, 2014, Employee #1 sustained a serious injury to her right ring finger as she reached into the Sheeter Head Machine to remove and clean excessive dough collected in and around the movable parts of the sheeter head assembly. She was transported by ambulance to Community Hospital of San Bernadine where she was treated for her injuries and released the same day. On February 12, 2014, Employee #1 had a follow-up procedure at Orange Community Hospital.
