Investigation Summary
At approximately 1:00 p.m. on January 27, 2014, Employee #1 was working at a facility that manufactured paper goods. He adjusted a paper roll (weighed approximately 1,200 lbs) that was suspended on the shaft of sheeter (Hamblet Machine Company, Number 788) and at a height of 8 ft above the ground. Employee #1 was using a portable stepladder to access the paper roll. He had to placed his left foot on the third rung of the ladder and his right foot on a piece of wood shaft used to secure rolls. His right foot slipped, and he fell on to a concrete floor sustaining a serious and unspecified injury. Employee #1 was transported to a hospital, where he was treated and then hospitalized.
