Investigation Summary
At approximately 2:30 p.m. on December 11, 2013, Employee #1, a 55-year-old male with Quad Graphics Inc., and was operating a (CATERPILAR, Model NRP-30, Serial Number INRPO763), Hathorne Lift Systems, powered pallet-jack, Number 11. As Employee #1 was backing up the pallet jack his left hand got caught between the railing and the handle of the pallet jack, which caused a fracture on his ring finger and amputation of his left pinkie finger. Employee #1 was hospitalized for three-days as a result of this event. The employer had total of 550 non-unionized workers and was engaged in commercial printing. Employee #1 had been employed, full-time worker with Quad Graphics Inc., for about one year prior to this event.
