Employee Struck by Container Falling from Forklift

At approximately 3:15 p.m. on December 27, 2012, Employee #1, a groundman, was assisting a coworker running a forklift, transporting a raised pallet carrying a cardboard carton of plastic pellets on top of a supersack. Unexpectedly, plastic pellets leaked out of a tear in a corner of the supersack, and Employee #1 stepped close to the supersack and put his hand up to stop the pellets from leaking out. The pellet leak caused the top surface of the supersack to incline from the horizontal, as a result the box slid off towards the leaking corner of the supersack, striking Employee #1's left shoulder and backside. Employee #1 sustained blunt trauma and a fractured pelvis. Employee #1 was taken to a local area hospital where he remained for several day.
