Driver Improperly Operating a Pallet-Jack Injures Himself

At approximately 8:15 a.m. on May 28, 2012, Employee #1, a 21-year-old male, and a scanner with Deploy Hr Incorporated, was working in the merchandize scanning area of the Scanning Department of the warehousing facility, in Fontana, California. Employee #1 was moving a large size pedestal fan (on wheels) and a "CROWN, model number PE4500-80-inch rider electric pallet jack, number 215, serial number 6A273587 was in his way. While standing on the floor, Employee #1 attempted to drive forward and inadvertently drove the pallet jack in reverse and his right lower leg got pinned between a six-inches wide and two-inches thick horizontal bar, of the guard railing and rider platform of the moving pallet jack injuring him seriously. Employee #1 sustained fracture of right lower leg. Employee #1 was taken to the Arrowhead Regional Medical Center, Colton, California, underwent surgery and was hospitalized for one week. Employee #1 was a regular employee of the employer. The employer is engaged in supplying work force along with their training including management officials for supervising merchandize receiving, storage and pulling customer orders, processing and preparing them for shipping out of the warehouse.
