Investigation Summary
At or about 9:00 a.m. on April 19, 2012, Employee #1 was trying to remove a stuck aluminum tube from the plastic core of a empty liner roll. With other employees who were assisting, all of whom were working for Erosion Control Applications Inc., Employee #1 attached one end of the tube to the Skid Steer frame (John Deere Skid Steer (Bobcat)), (Model 325, JF7) via a nylon strap and the other end to gradall (boom lift). Employee #1 was on foot on the ground under or very close to the elevated bucket. As the Bobcat operator lowered the bucket and backed it up to pull the aluminum tube, the bucket struck Employee #1 in the head and pinned him causing fatal injuries. The site foreman did not train and provide specific instructions to the operator of the Skid Steer, to warn and communicate with all the involved employees prior to backing up.
