Investigation Summary
At approximately 2:00 p.m., January 30, 2012, Employee #1, a machinist employee of Mitco Industries Inc., was seriously injured while operating a lathe machine, located in Lathe Dept. of the manufacturing facility. At the time of the accident, Employee #1 was operating a "HITACHI SEIKI, model 5A" manually operated Turret lathe machine. Employee #1 set a 9 ft. 3/4 long by 2 ft. 5/8 inches diameter metal part (known as Ampco 45) in the lathe chuck and inserted a chuck key in its' hole to tighten the part. Employee #1 was turning the key in clockwise direction with his hands when his left arm inadvertently pressed the chuck spindle engagement lever. This activated the chuck to move down at which time Employee #1's left hand got caught between the key handle and the splash guard. He sustained amputation of his left little and ring fingers. The chuck spindle engagement lever was not maintained in safe operating condition, in that the lever was not installed and located in a manner such that it cannot operate from accidental contact with objects or parts of the body. Employee #1 was taken to the Arrowhead Regional Medical Center, Colton, CA for treatment. Surgery was performed.
