Investigation Summary
On May 24, 2012, Employee #1 was assisting the truck driver of a tractor-trailer truck. The trailer was approximately 60 foot long, with a bumper approximately 4 foot high, with an obstructed rear view of the receiving dock at a flooring retail store. After talking to Employee #1, the truck driver went back to the cab and pulled forward from the dock. Employee #1 walked down the stairs to the right-side of the truck (the opposite side from the driver) to apparently lift the dock plate up. At the same time, the driver of the semi-truck stated that he was told to back up. Once the driver backed up, Employee #1 became caught between the dock plate and the truck. The driver exited his cab and walked around to the right side to enter the building. When he got about half way, the driver noticed that Employee #1 was pinned. The truck driver pulled forward in order to free Employee #1. Upon freeing Employee #1, the truck driver ran to the office in the front of the building. Employee #1 followed him into the office and subsequently passed out once he got to the office. Employee #1 died later that day from the crushing injuries to his upper body.
