Investigation Summary
On March 7, 2012, Employee #1 died after being struck and crushed by a structural steel beam. At the time of the incident, a forklift truck had just loaded the beam on to a flat bed trailer. After Employee #1 released the rigging from the beam, he climbed down from the bed of the trailer. The steel beam then fell from the trailer and struck and crushed Employee #1. Employee #1 was pronounced dead at the scene of the accident.
