Employee Suffers Fatal Fall from an Elevated Height

On December 22, 2011 Employee #1 of Horseshoe Bend Improvements was installing copper gutter to a home. Employee #1 was working from a ladder jack scaffold in which the working platform was approximately 21 feet 9 inches above a brick patio. Fall protection was not being used. Employee #1 lost his balance and fell to the ground. He suffered a severe head injury and passed away at 11:42 p.m. on December 31, 2011.
