Employee Dies after Being Struck by Falling Tree

At approximately 7:00 a.m. on November 1, 2011, Employee #1 was assigned as a spotter, to communicate by hand signal when the tree was ready to be pulled down. After Employee #1 gave the sign that the tree was ready to be pulled, he started running towards the tree being pulled down, and when the foreman realized that Employee #1 was between the tree and the truck, he stopped the truck and told him to leave the danger area. The rope became slack due the truck not pulling the tree, and the tree fell down and hit Employee #1 in the shoulder area. Employee #1 died at the hospital from his injuries.
