Employee's Finger Is Amputated by Machine

On August 25, 2011, Employee #1 was freeing jammed material in the leg of a shakeout. He locked out the machine, attempted to clear the jam, reenergized the machine, and started it. When Employee #1 determined that the machine was still jammed, he reached towards the belt and pulley to manually move belt. His right index finger was amputated.
