Investigation Summary
On April 30, 2013, Employee #1 was working in an area where trees were being cut down. A coworker gave the signal to the employee that the tree was falling his direction. Employee #1 did not move when tree was cut. The felled tree struck the employee breaking his neck. Employee #1 was pronounced dead at the scene.
