Employee Falls Off a Roof and Sustains Fracture

On May 21, 2012, Employee #1, with Horizon Builders Inc., was installing a tarp on an unfinished roof as waterproofing, and he stepped onto the other side of the roof and slipped and fell 15 feet to the lower ground. Employee #1 was not wearing fall protection. He was hospitalized with an unspecified fracture.
