Employee Is Fatally Struck by a Pipe

At approximately 10:00 p.m. March 3, 2012, Employee #1 of J M Manufacturing Co Inc., was performing maintenance in the area of the Line 1 Unitizing Rack. Pipes continued to feed onto the Secondary Storage Rack while he worked, and eventually a pipe weighing approximately 380 pounds was forced off the table and fatally struck the victim.
