Investigation Summary
Employee #1 was finishing concrete on the bottom of a newly constructed swimming pool. The crew had just finished two sides, and the cement driver was instructed to add some water to the concrete. After adding water, the truck became unstable, along the side of the pool, causing the concrete truck to roll over and fall into the pool. Employee #1 was crushed as a result of the accident, and died from his injuries.
