Employee Crushed By Loader

At approximately 2:00 p.m. on July 20, 2011 Employee #1 and a coworker were working on a cattle shed sliding door system. The overhead track was in need of repair. The coworker n the sliding door on the ground and Employee #1 was trying to get a birds nest out of the track. Employee #1 was lifted in the bucket in place of a ladder to reach the verhead track. The wheel loader had rolled into a shallow rut or depression making it difficult for Employee #1 to reach the overhead track. When the coworker released the brake, the equipment must have been in gear and lurched forward. To quickly stop the equipment from moving, the coworker stomped on the brake, but was confused and stomped on the gas pedal. Employee #1 was crushed between the bucket and the header of the cattle shed, then thrown from the bucket hitting his head on vertical member of a concrete feeding trough and finally landing approximately 50 ft in front of the wheel loader. Employee #1 never regained consciousness and was pronounced dead at the hospital.
