Investigation Summary
At approximately 4:57 p.m. on July 13, 2011, Employee #1, of Tapscott Brothers Logging Incorporated, went to the tire storage trailer with a forklift to pick up some tires. When he didn't return, a coworker went to check on him and found him pinned between the forklift and the door of the tire trailer. The medical examiner declared him killed at the scene.
