Employee Is Killed When Crushed by Auger in Paving Machine

On July 18, 2011, Employee #1 was working on a street repaving project. Employee #1 was working alone and was conducting routine maintenance on a Power Paver SF-3000 street paver machine. Employee #1 was greasing two 18 inch horizontal augers with four grease points while the machine was running. The machine can run without the augers moving, and a separate control is used for the augers. Lockout/tagout can be used on this machine, but Employee #1 did not utilize lockout/tagout. Greasing the augers is a job that normally requires two employees, one to grease and one to operate the controls that turn the auger on and off. Employee #1 performed this job alone. First Employee #1 greased the auger, then he ran the auger. Employee #1 repeated this sequence two or three times until grease was observed at the end of the auger cylinder. After two or three attempts to grease the auger, Employee #1 left the auger running while trying to grease it and his clothing was caught by the auger. Employee #1was pulled in to the auger and was crushed.
