Investigation Summary
Employee #1 was walking through the workplace area where welding operations were being performed upon a standing steel rectangular truss structure that was resting on the workplace concrete floor. The structure was stabilized at each end by a shop made saw horse device, resting upon the floor and attached to the truss structure. The surfaces of the floor, and the saw horse floor, plates were smooth. The saw horse devices were not attached to the floor; nor was the truss structure held in place with overhead lines. The standing truss structure suddenly moved from the vertical to the horizontal position striking Employee #1 as he was passing through the area.
