Investigation Summary
On April 1, 2012, Employee #1 was stacking stoners (metal bins used to store graphite) in a storage area. While stacking these stoners, one of the stoners had tipped into a lower stoner. In an attempt to correct the tipped bin, he used two powered industrial trucks. These powered industrial trucks were two identical stand-up Hysters and used in tandem by Employee #1. He would brace the bins with one Hyster and then dismount the forklift and would then mount the other Hyster to maneuver the tipped bin. At some point, the employee parked one of the Hysters behind the one he was using to correct the tipped bin. Employee #1 left the forks on the parked Hyster about 3-feet above the floor. As he was maneuvering the front forklift truck, he drove the forklift in reverse and punctured his leg on the raised forks of the parked forklift. Employee #1 had walked to the phone to call for help; however, he lost consciousness prior to doing so. Employee #1 was found later by a fellow employee who had found him at the telephone.
