Investigation Summary

At approximately 7:30 p.m. on February 13, 2012, an employee was discovered by a coworker lying face down beside his car on the 4th level of the parking garage at the employer's facility. The employee was reported to be an engineering consultant and long term employee with the company. The coworker reported the finding to security at the facility, who initiated emergency response communications and summoned emergency services. At approximately 7:48 p.m. EMS arrived and found the employee unresponsive and began treatment. EMS moved the employee from the 4th floor of the garage to a waiting ambulance at approximately 8:48 p.m., and the employee was transported from the facility at approximately 9:05 p.m. and taken to the hospital. The property manager and facilities manager proceeded to the hospital, where they met the employee's next of kin, his wife. After attempts to treat the employee, the doctor reported at approximately 10:15 p.m. that the employee had died from heart related causes.
