Investigation Summary
On May 27, 2011, five employees were trying to determine which of 3 pipes in a sub-surface trough containing Hydrogen, Nitrogen and Compressed air was leaking near Annealing Oven Number 1. They attempted to remove the South trough cover using a pry bar but it would not move. Then they went to the North side and removed that cover with an overhead crane. After removing the cover, they still could not access the area of the leak so they returned to the South side cover. One of the employees brought over a forklift and a chain was hooked from the forks to the trough cover. Apparently as the cover was being lifted an explosion and fire occurred. Five employees were injured and transported to various hospitals. Two employees were treated and released. Two employees subsequently died a few days later. One employee remains hospitalized in critical condition.
