Employee Dies After Being Pinned By Forklift

On November 1, 2011, Employee #1, a maintenance person, was operating an electric rider pallett jack to move materials in the shipping and receiving bay. Employee #1 fell off the pallet jack and ended up being pinned between the pallet jack and a round metal support beam. Employee #1 suffered lacerations to the thoracic aorta, bilateral hemothorax, multiple rib fractures, fracture of thacic spine, and sternal fracture. Employee #1 died later in the evening, due to his multiple injuries.
