Employee Adjusts Machine Rotating Part and Amputates Finger

On May 23, 2011, Employee #1 had just finished with the setup for the Spin Forming Machine. The machine was not trimming the rims adequately, so he adjusted its rotating cutting part while it was running. As he did this, Employee #1's right hand contacted the part and was pulled into the machine, resulting in an index finger amputation. He was taken to a medical center, where he underwent surgical procedures and was then hospitalized for postoperative care.
