Investigation Summary

Investigation Nr: 200375749
Event: 08/31/2011
Employee Is Killed When Caught Between Textile Equipment

At approximately 3:00 a.m. on August 31, 2011, Employee #1 was operating a sanforizer machine, which was used to treat cotton fabrics to reduce shrinkage when washed. The machining process included the stretching, shrinking and fixing of the woven cloth before the cutting and producing phase. Employee #1 had been talking with his supervisor about some quality issues with the fabric. Upon the completion of the discussion, Employee #1 returned to his duties near the machines S-wrap roller. For an unknown reason, Employee #1 became entangled in the machine's roller system. When the Supervisor returned fifteen minutes later, Employee #1 was discovered unconscious and caught between the roller. Emergency services were contacted and, upon their arrival, Employee #1 was determined dead as a result of positional asphyxia.
