Investigation Summary
At approximately 9:55 a.m. on September 3, 2011, two employees were working on a 30 foot high roof without any fall protection. Employee # 1 and Employee # 2 were taking measurements and painting marks prior to installing a warning line system. Employee # 2 was holding the tip of the measuring tape and employee # 1 walked backwards with the measuring tape. Employee # 1 walked backwards off the roof and fell 30 feet to the ground and died at the scene from his injuries.
