Investigation Summary
On January 13, 2012, Employee #1 was working as a service technician that was dispatched to repair a heating unit and the rooftop. Employee #1 fell from the ladder while trying to gain access to the roof top and fell approximately 20 feet 1 inch to concrete below. Employee #1 was transported to the hospital, where he was hospitalized and treated for approximately 12 days. Employee # 1 later died.
