Employee Dies at Excavation Site

On October 7, 2011 Employee #1 was working beside an excavation. Blown cellulose insulation was being vacuumed out of a barn attic by a coworker and fed by a hose to the excavation. The roof of the barn was to be removed and the barn expanded onto a larger concrete pad and footings, and the new roof installed. As the insulation was blown into the excavation and subsequently onto the surrounding area, this made it difficult to see the edges of the excavation, Employee #1 at some point fell into the excavation and cellulose insulation and was unable to get out. Employee #1 became trapped and covered by the insulation and died as a result of asphyxiation
