Investigation Summary
On April 27, 2011, Employee #1 was retiming a Model Number 8400 Diamond System Egg Grader. Employee #1 went to the side of the machine to turn the main drive to the "0" setting on the dial. A coworker pushed the "START" button on the machine that had not been locked out. The machine started, and when the belt began to turn, it amputated Employee #1's right thumb. Employee #1 was hospitalized.
