Investigation Summary
On September 4, 2012, Employee #1, a 48-year-old male with the Smithfield Packing Company Inc., was working on 120-volt smoke generator number eight, wiring conduit to a breaker panel. The smoke generator was not locked out; Employee #1 was working on the smoke generator when he received an electric shock. The electric shock caused Employee #1 to drop into unconsciousness and later death.
