Employee Gets Caught in Conveyor

On March 2, 2012 Employee #1 of Stein, Inc., was working around a materials-handling conveyor (4' wide belt) located in the slag area of a steel plant. It was unclear what or why was being done, but Employee #1 was pulled into the conveyor when it was started. Employee #1 was crushed between the conveyor belt and the frame of the conveyor.
