Investigation Summary
On June 13, 2011, Employee #1 was removing a canvas awning from the balcony of a third-floor condominium. Employee #1 was reportedly standing on the balcony's deck, and was not on a ladder, nor was he standing on the balcony's railing. He pulled on the canvas and fell over the railing. Employee #1 fell approximately 30 feet to the ground and was killed.
