Employee Falls from Forklift, Fractures Leg

On June 8, 2013, Employee #1, with Pctc Cabinetry Inc., left a forklift he was operating with its forks raised about 18 in., and then he proceeded to get on a second stock picking lift and drove it. Employee #1 drove this second lift into the raised forks of the first forklift. His right leg became entangled between the forks and his industrial truck, causing him to fall. Employee #1's leg was caught between the raised forks of the lift and the truck. Employee #1 was hospitalized with a fracture to his leg.
