Investigation Summary
On May 6, 2013, Employee #1 of Christiansen Amusements Corporation was seriously injured. Employee #1 was dismantling a Kamikaze ride and was attempting to remove some 30 mm bolts, but was turning them the opposite way of removal. He then tried to remove the bolt by using an extender, lost his balance, and fell approximately nine feet to a dirt surface. Employee #1 suffered fractures to his left wrist and femur.
