Investigation Summary
On November 14, 2012, Employee #1 of Jellco Container Inc., was working at the in-feed section of a cardboard box folding machine. The cardboard material is fed into the in-running rolls of this machine. At the time of this accident, a batch of cardboard boxes was just been completed and the last batch had been fed into the machine. While the machine was still running, Employee #1 proceeded to perform the setup and the servicing operations to get ready for the next batch of boxes to be processed. A part of this operation required him to apply wax to the side guide plates at the in-feed to facilitate the easier movement of the material into the machine. He was rubbing a small block of wax against the side guide when his right hand was pulled into the in-running rolls at the feed point of the box folding machine. His right arm was pulled into the machine up to his elbow. Employee #1 suffered multiple fractures to his right arm and de-gloving injury to his right hand. He was treated and hospitalized for six days.
