Investigation Summary
On July 30, 2012 Employee #1, a 55-year-old male with Titan Sheet Metal Inc., was installing some sheet metal on top of a two-story residential building. Employee #1 was about to come down an extension ladder to get some more material. As he stepped onto the ladder, it slipped away from under him and he fell down approximately eighteen-feet to the dirt surface on the side of the building. Employee #1 suffered multiple fractures to his pelvic area and was hospitalized for four days. Employee #1 was not a contract employee. This employer was engaged in the installation of sheet metal parts to the residential and commercial buildings.
