Investigation Summary

Investigation Nr: 202555421
Event: 09/14/2012
Employee Is Injured in Fall from a Stepladder

On September 14, 2012, Employee #1 was making repairs to a motor that was located 9-ft above the floor adjacent to the conveyor in the bakery production area. The employee was standing on the third step from the top cap of a 6-ft portable stepladder when he lost his footing and fell. It is unclear whether he hit his head on the floor or whether the ladder struck him when it fell with him. After the fall, Employee #1 was conscious but dazed and confused. The employee was transported to UCI Medical Center, where he was admitted for surgery for fractures to his frontal lobe and brain contusion.
