Investigation Summary
On May 5, 2012 Employee #1 was cutting a piece of lumber with a skill saw. As he was making the cut, the saw blade kicked back and amputated Employee #1's left index finger and thumb. Employee #1 was taken to a local area hospital, where he stayed for several days.
