Employee Gets Finger Amputation by Saw Blade

On November 17, 2011, Employee #1 was operating a jet equipment and tool's model WBS-24-3 band saw, serial # 30163458. The band saw's guard not adjusted in close proximity to the stock being cut. As he cut a frame from the plastic sheet, his left hand struck the saw blade and his left middle finger was amputated.
